- IU Health is now offering charge pay, which is a $3.00 bill
rate bump per hour

- Department Details: 5C is a 31 bed med-surg unit with a
concentration in general surgery, urology, gynecology and bariatric
surgery. Must have 3-4's in Bariatric Surgery, Thyroidectomy, Bowel
Obstruction, and CBI
- Patient Ratio: 1:6
- Effective 9/1, the COVID vaccine is required for all travelers
at IU Health campuses. Declinations will not be accepted; however
medical or religious exemptions will be accepted with a doctor's
note provided.

- Unit Specific Information
- 5C is a 31 bed med-surg unit with a concentration in general
surgery, urology, gynecology and bariatric surgery. Must have 3-4's
in Bariatric Surgery, Thyroidectomy, Bowel Obstruction, and CBI
- Additional Details
- Effective 11/30/24, IU Health will not allow IN RN's to work at
their facilities (SEE EXCEPTIONS BELOW). The last day for IN RNs to
start a NEW contract is 9/9 (allows them to finish a contract by
11/30).
- **Periop/OR RN's, Allied and Ambulatory RN travelers are exempt
from this rule. These IN travelers are allowed to work past the
11/30 deadline. Periop includes ENDO, IR RN, PACU, Cath Lab RN,
SDS**
- Valid Driver's License or Passport accepted. Must be clear
copy.
- AHA ONLY. RQI is acceptable.
- Please check the cost center to verify what certifications are

- HIPAA Acknowledgement required.
- Online Verification of License to include expiration date and
any disciplinary actions (within 30 days before start date). Must

- LICENSING: Task the Program Manager for approvals on license
issues.
- Medication Test/Pharmacology exam required.
- Not required for Techs or RRTS
- Photo for Badge required asap.
- The Look of Assurance packet must be sent to all candidates for
review and initial via DocuSign prior to start.
- Complete facility specific forms as a priority.
- Proof of valid auto insurance required for all travelers under
Hospice specialty.
- Required for all travelers going to Indiana University Health
Physicians only.
- EPLS/GSA/SAM required within 30 days of start
